Awesome, very helpful. We use it mainly for post-purchase surveys on the thank you page ("how did you hear about us", "why did you buy product XYZ") and overall purchase satisfaction sent automatically via email ("how satisfied are you with your recent purchase"). It's reasonably easy to use but most of all, it's just really reliable and versatile. We made some of our core business decisions based on the survey results we received through this app. Oh and the founder Jason is just awesome!
